Summary:
Provides oversight, supervision, direction, scheduling, and quality assurance for the triage team. This position will report to the Director of Crisis services, and will be key in providing oversight of this important crisis service. Half of the time the team lead will focus on providing direct triage service as a team member in triage, while the other half of the time the team lead for triage will focus on supervision of staff, oversight of quality related reports including disposition reports, scheduling, and quality items.

Facilitates communication between the statewide call center and mobile crisis program to provide efficient response to request for assessments. Provides program support to promote quality and consistent service delivery.
ESSENTIAL FUNCTIONS:

· Provide bi-weekly supervision of the triage team members

· Annual evaluations

· On-boarding, orientation to triage team for new employees

· Working closely with the Director and Senior Director of Crisis as needed or identified to address internal and external challenges

· Oversight of monthly schedule

· Work with Director, and Senior Director to ensure high quality services are provided on the triage team as it relates to referrals/discharges to both adult/child CRU programs

· Ensures quality daily follow up and reporting relating to Crisis Contract with both admissions and discharges from both adult/child CRUs.

· Provides oversight, and direction for follow up phone calls occurring with discharges from Adult/Child CRUs, consistent with crisis contract

· Provides oversight of the completion of any and all reporting requirements associated with state crisis contract, specific oversight of template used to track admissions, and denials to both the adult/child CRUs

· Triages requests for assessments and client needs for program.

· Facilitates communication between the Statewide call center and mobile crisis program to provide efficient response to requests for assessment.

· Identifies and assigns program staff to assessments and other community needs.

· Monitors high risk cases.

· Tracks follow up efforts and encourage consistency with the components of the Golden Thread.

· Serves as a point person for communication with internal/external providers.

· Manages and maintains staff schedules to ensure appropriate staffing levels for the program.

· Serves as gatekeeper for referrals for admission to Crisis Stabilization Units.

· Takes a role in the training and orientation of new staff.

· Collaborates closely with Director and Supervisors around the programs and implements directives.

· Develops and fosters relationships with community providers.

· Supports in promoting overall success and functioning of the team.

· In collaboration with the caller, determines the need for a face-to-face assessment and, if needed, negotiates a meeting site convenient for the individual that provides safety for all involved.

· Conducts safety screening with callers.

· Aims to provide service in the least restrictive setting that supports the comfort and dignity of the person served.

· Communicates directly with persons experiencing a crisis in order to assess mental status, current level of functioning, and the person’s wishes and goals for crisis stabilization.

· May request protective custody interventions by law enforcement personnel in order to resolve potentially life-threatening situations.

· Conducts program data entry and assist in reporting and interpretation.

· Develops and participates in implementation of crisis follow-up by providing stabilization for up to 30 days.

· Provides necessary information/communication to meet all requirements needed for managed care authorization of services and 3rd party insurance reimbursements.

· Meets recordkeeping requirements by the timely completion of assessments and by maintaining current progress notes, logs and other information as required in the client’s chart.

· Ascertains that relevant information is reported to program manager and/or the team in a timely manner.

· Assure communication with treatment team at time of assessment.

EDUCATION:

· Bachelor’s Degree preferred.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

· MHRT/CSP

· Valid state driver’s license.

EXPERIENCE:

· Minimum of two years relevant experience required.

· Six (6) years of directly applicable crisis intervention experience may be accepted in lieu of completed Bachelor’s Degree plus two years of experience at the hiring manager’s discretion.

KNOWLEDGE AND SKILLS:

· Recognize signs of increasing clinical or situational risk and of escalating behavior and utilize de-escalation and safety implementation techniques effectively.

· Recognize variables underlying stressful situations, discern methods to diffuse or contain them, and quickly implement the most effective intervention.

· Manage multi-task situations and conflicting demands, cope with stressful situations, prioritize projects and responsibilities, and meet critical deadlines.

· Use listening skills, flexible collaboration and a consistent problem-solving focus.

· Communicates ideas and opinions in a clear and constructive manner, both verbally and in writing.

· Intermediate computer skills required.
