POSITION DETAILS:
• Offer trauma informed other best-practice clinical techniques and therapies.• Work with hospitals, crisis stabilization unit’s (CSU’s), or residential treatment programs around possible step downs from higher level of care to lower level of care.• Provide customer service within established deadlines, consistent with organizational values, standards, and policies.• Demonstrate leadership in case-coordination.• Complete timely, professionally written, intake assessments.• Schedule clinical appointments to meet the clinical needs of the client. • Work with hospitals, crisis stabilization unit’s (CSU’s), or residential treatment programs around possible step downs from higher level of care to lower level of care.• Use organization’s established tools and procedures to formulate assessment. This includes level of care and risk assessments as needed. Involve family members as needed to establish a diagnosis.• Develop an individualized service plan with the Child(ren) and family.• Develop interventions to be utilized by the team, including the child and family crisis aftercare support specialist.• Identify concrete service needs and provide integrated treatment with the client’s natural supports and providers to demonstrate best-practice.• Ability to work on-call shifts, including weekend and overnight shifts. Be willing to work a flexible schedule based on client’s needs.• Address barriers to care including transportation, financial, family support, lack of service providers, and physical health.• Document services by entering participant data into (EHR), complying with all organization, state, and federal documentation requirements.• Implementation of data tools in accordance with contractual agreement.• Responsible for the implementation of the individualized crisis management plan that promotes less restrictive alternatives to hospitalization.• Attend to the emotional, behavioral, and physical health needs of the child(ren) in a holistic client-centered manner.• Support children and their families to navigate individual needs that assists in obtaining services that are required.• Refer to other services including, but not limited to, other behavioral health and/or medical services, housing, food, clothing and other basic needs.
EDUCATION REQUIREMENTS:• Master’s degree in Social Work or Counseling with a concentration in clinical practice from an accredited graduate school.
CERTIFICATES, LICENSES, REGISTRATIONS:• Appropriate licensing certificate (LCSW, LCPC) preferred.• LMSW-cc, LCPC-cc, at minimum required.• Maintain a valid state driver’s license.
